Additional Corporate Member Broker Form – This form should be used to add a broker/employee to a YBAA Corporate Membership. This is NOT a membership application form.

Please add broker's name, company name and address, direct line/cell phone, e-mail and signature. All the brokers associated with your company who will be designated to receive all benefits of YBAA membership, including voting privileges, as long as the Corporate Membership is in effect, must complete this form. Each broker must provide their electronic signature, in the space provided, indicating that they will attest and subscribe to the YBAA Code of Ethics.
